Transcript Request
All former students whom would like to request an Official copy of their High School Transcript are now required to fill out the Official Department of Education Transcript Request Form and bring proof of ID for every transaction. Please click here for the document. Acpolicytranscriptrequest.pdf


Forms can be faxed to ATTN: Samantha at (212) 242-7630 or you can come to the school to personally to fill out a form. Please indicate on a seperate sheet, the exact address the transcript needs to be sent to or if you will personally come to pick up the document. Please include your phone number for questions in regards to the request.

Note: All Requests may take up to 5 days to be fulfilled.
