1. REVIEW THE PROVIDER DIRECTORY: Carefully review the provider information. Try to select a program that best meets your child’s academic needs. Consider the kinds of programs offered, group size, staffing, the days, time, frequency of service and the location of the services. If you need help, you may ask your child’s school for a recommendation. But remember that the final decision is up to you!
2. CONTACT PROVIDERS: Get in touch with providers by phone, email or arrange a visit to the provider site or center. If your child’s school holds an information session with providers, be sure to attend. Please be sure to bring the enrollment form, and information about your child’s performance (report cards, Grow Reports, etc.)
If you are not a native English speaker, be sure to ask about the provider’s capacity to provide information in your home language. It is important that the provider that you select is able to communicate with you about your child’s progress and to contact you in case of an emergency.
3. COMPLETE THE ENROLLMENT FORM: Once you have selected a provider, write in the provider name, provider ID and the program service code number (as listed in the provider directory); the provider may assist you with this to ensure the correct information is entered. Read the consent statement and sign/date the form. Write in your current contact information for the provider.

4. SUBMIT THE ENROLLMENT FORM: Submit the completed enrollment form to your child’s school or to your selected provider.
Once the form is completed and signed, the selected provider will submit it to the SES Program Office for processing. Be sure your completed form is submitted by the deadline so that your child may begin receiving SES services in a timely manner.

THE NYCDOE WILL ONLY PROCESS ONE ENROLLMENT FORM PER CHILD.
5. NOTIFICATION: Once all forms are submitted and processed, providers will receive a notification of final approval and contact you. Remember that, in accordance with the Federal law, if there are not sufficient funds to grant all requests for SES, we will give priority to requests from eligible students with the greatest academic need.
6. START OF SES PROGRAM: The provider you selected will contact you with specific information for the start date and location of your child’s program. Not all programs will begin at the same time. If you do not hear from the provider, please contact the provider and notify the school. If at all possible, you are encouraged to accompany your child on the first day services will be provided. At that time, your child’s Supplemental Educational Services Plan will be developed by you and the provider. The provider must give you a copy of the plan and will send a copy to your child’s school.
