NEWS FROM THE DOE
The 2008 Fall Transfer Period will be open to Department of Education employees from November 3 through November 28, 2008. During this Transfer Period employees may change plans, add or drop a rider, etc., with an effective date of the first day of the first full pay period in January 2009.
School-based employees (Q and E Banks) must fill out an ERB Health Benefits Application, available at http://schools.nyc.gov/Offices/DHR/DHRForms/default.htm, and sign, date, and submit it during the Transfer Period, between November 3 and November 28, 2008, along with any necessary supporting documentation.
Administrative employees (H Bank) must call HR Connect at (718) 935-4000, state their intention for the Transfer Period and provide their Employee ID number. The DOE will then generate and mail the employee a customized health benefits enrollment form within 5-7 days. The employee must sign, date, and submit the customized form during the Transfer Period, between November 3 and November 28, 2008.
